Ohana Participant Information

We are glad you are going to be participating in the 2014 Jump Start: Ohana Program which runs from August 16-21. On this page you will find links to six very important documents. The participant letter gives you important information about the schedule on the first day. The Information Response form helps us plan how best to assist when you arrive to campus. The Liability/Health and Release forms are very important to prepare for a safe and fun program. A Check List and Maps are for you to use.

Please complete all of the forms and mail back to ATTN: Office of Multicultural Affairs, 900 State Street, Salem, OR. 97301.

Please let me know if you have any questions. We have a great program planned and look forward to getting to know you!

For more information, feel free to contact me or browse the Jump Start or Ohana websites;
