To submit a request for new keys or for lost or stolen keys:
- Download the appropriate form below and fill it out.
- Obtain the required signature(s) and then submit the form to the Facilities Management Office via email at facilities-info@willamette.edu
- Once the keys are ready for pick up the Facilities Management Office will email you.
