All employees are encouraged to get a vaccine. This year we will have a limited number of additional shots that can be provided to dependent children and spouses. Employees and eligible family members must be age 65 or below to participate. All Kaiser Insurance members must be age 18 or older.
Salem Campus - Thursday, Oct. 19, 9 a.m.–2 p.m. in Henkle Gym West (lower level of Sparks). (Sign-ups close on Tuesday, Oct. 17, at 3 p.m.)
Portland Campus - Friday, Oct. 20, 10 a.m. - 1 p.m. in the Hammer Conference Room. (Sign-ups close on Wednesday, Oct. 18, at 3 p.m.)
An appointment will be required this year (walk-ins will not be allowed).
Please schedule your flu shot appointment through WISE and click on the tab labeled "Sign-up," on the left side of the page.
To Receive A Shot, You Must:
- Bring the completed Insurance Claim Form and your insurance card to your vaccination appointment;
- Be age 65 or younger;
- Be age 18 – 65 if you are a Kaiser Insurance member (if not insured by Kaiser, the clinic can provide shots to those as young as 9 years of age);
- Not had an anaphylactic-type reaction to latex;
- Not have an allergy to chicken eggs and/or egg products;
- Not have an allergy to Thimerosal (used as a preservative in vaccines);
- Not be exhibiting symptoms other than mild coughing, runny nose, and/or diarrhea;
- Not have a history of Guillain-Barré Syndrome;
- Not had a serious reaction after receiving a prior influenza and/or pneumonia vaccination;
- Meet any CDC eligibility requirements required at the date of service;
- Ensure you are dressed appropriately so the nurse can reach your upper arm;
- *Clinic providers encourage you to have a snack and come hydrated
